The short answer

Return to sport should be decided by tests, not feelings or calendars. "It feels fine" are the three most expensive words in sport. Every physiotherapist has met the athlete who felt fine, played on a Sunday, and spent Monday booking a scan.

The problem is that feeling recovers faster than capacity: pain switches off weeks before strength, power, and control return, and the gap between the two is exactly where re-injuries live. Modern rehab closes that gap with objective testing, a set of measurable gates an athlete passes before being cleared.

This article walks through the five tests we use, what passing looks like, and why the athletes who test patiently end up playing more sport than the athletes who rush, whether the injury was an ankle sprain or an ACL reconstruction.

Why "feeling ready" fails as a test

Feelings report symptoms; sport demands capacity. After an injury, pain settles as inflammation resolves, but the deeper losses, muscle strength, tendon stiffness, reactive control, and sport fitness, recover on a slower clock and only if trained.

Research across injuries keeps finding the same pattern: athletes routinely carry 15 to 25 percent strength deficits at the moment they feel completely normal. The nervous system cannot feel a strength deficit at rest.

It discovers it in the 80th minute, mid-tackle, on landing, and that discovery is the re-injury. Testing exists to find the deficit in the gym, where finding it is free.

Test 1: strength symmetry, the foundation

The first gate is simple: the injured side should produce at least 90 percent of the force of the uninjured side in the muscles that matter for your injury and sport. After knee injuries that means quadriceps and hamstrings; after ankle sprains, the calf and the muscles that guard the ankle; after shoulder injuries, the rotator cuff and scapular muscles.

Measurement can be done with dynamometry or with practical gym-based proxies like single-leg press and repetition-maximum testing. Two honesty notes.

First, the uninjured side often weakens during rehab too, so comparing against a diminished baseline flatters you; where pre-injury data exists, use it. Second, 90 percent is a floor, not a target; many clinicians push athletes in cutting sports toward full symmetry before high-risk return.

Test 2: hop and power tests, strength at speed

Strength is force; sport is force produced fast. The hop battery tests that conversion for the lower limb: single hop for distance, triple hop, crossover hop, and a timed 6-metre hop, each compared side to side with 90 percent symmetry as the pass mark.

Landing quality is scored alongside distance, because a long hop with a wobbly, stiff-kneed landing is a fail wearing a pass's numbers. For upper limb athletes, throwing velocity and medicine ball tests play the same role.

Power tests are also where confidence quietly shows itself: an athlete who shortens the hop on the injured side is protecting, and the protection pattern itself raises re-injury risk.

Test 3: sport-specific fitness, because fatigue is the trap

Injuries cluster at the fatigued end of matches, and a large share of re-injuries happen when a deconditioned athlete meets full match intensity. Weeks of rehab erode the exact fitness your sport demands: repeat sprint ability for football and cricket, rally endurance for tennis and badminton, sustained pace for distance running.

So the third gate is rebuilding it before return: graded conditioning that recreates your sport's work-rest pattern, measured against pre-injury benchmarks where they exist, like a repeat sprint time or a yo-yo test level. The rule of thumb: complete at least one or two full-intensity training weeks before the first match.

Test 4: movement quality under fatigue and chaos

A clean single-leg squat in a quiet clinic at 10am proves little about a rebound contest in the last over of a tournament. The fourth gate examines how movement holds up when conditions degrade: cutting and landing mechanics tested after a fatigue protocol, reactive drills where the athlete responds to unpredictable cues rather than rehearsed patterns, and sport-specific chaos like contested drills.

What testers look for is whether the knee still tracks well, the trunk still controls itself, and the athlete still commits fully when tired and surprised. Movement that survives fatigue is rehearsal for sport.

Test 5: the head, the gate most programs skip

Psychological readiness is not soft science; it is one of the stronger predictors of who returns successfully and who re-injures. Fear of re-injury changes movement measurably: hesitation into tackles, weight shifted off the injured leg, cutting angles rounded off.

Validated questionnaires exist that score confidence and can flag an athlete whose body passed every physical gate while their trust in the limb has not. The fix is rarely a pep talk.

Confidence is built the same way strength is, through graded exposure: progressively harder, more sport-like challenges met successfully until the limb has earned trust back. An athlete who passes four physical gates but hesitates in contested drills is not ready, and playing them anyway tends to prove it.

How the decision actually gets made

Return to sport is not one yes at the end; modern practice frames it as three stages: return to training, return to play, and return to performance, each with its own bar. The decision is shared between athlete, physiotherapist, coach, and where relevant the doctor, weighing test results against context: the importance of the match, the time of season, the athlete's history, the sport's risk profile.

A club cricketer with a first-time ankle sprain and 88 percent symmetry might reasonably return for a final; a young footballer nine months post ACL with the same number waits. Criteria inform the decision.

They do not replace judgement, but they stop judgement being a mood.

What passing does not promise, and what it does

Honesty matters here: no test battery makes re-injury impossible, and anyone promising a zero-risk return is selling something. What structured testing does is shift probability meaningfully in your favour, and the evidence, clearest in ACL populations, shows athletes who meet discharge criteria re-injure at substantially lower rates than those who do not.

Testing also does something quieter: it converts an anxious guess into a plan. "Not yet, because your quad is at 82 percent, and here is the six-week block that closes it" is a very different conversation from "give it a few more weeks and see".

What this looks like in practice

A typical testing session runs an hour or so: strength testing for the relevant muscle groups, the hop or power battery with landings filmed, a conditioning benchmark, movement screening under fatigue, and a readiness questionnaire, finishing with a traffic-light summary per gate and a program for anything amber or red. Re-testing every few weeks turns the gap into a countdown instead of a mystery.

When the tests say not yet

A failed gate is information, not a verdict, and how it is handled separates good programs from demoralising ones. Each amber or red result should convert directly into a training block: a quad at 82 percent becomes six weeks of targeted heavy strength work with a re-test date; a shaky fatigued-landing score becomes a plyometric and control block; a low confidence score becomes graded exposure through progressively harder sport drills.

The re-test date matters psychologically as much as physically, because an athlete waiting for a vague "when you are ready" loses motivation, while one chasing a number on a calendar trains with purpose. Most gaps close in four to eight weeks of specific work.

The athletes who struggle are rarely the ones with the biggest deficits; they are the ones whose programs never measured, so nothing ever visibly closed.

The bottom line

Rehab without testing ends in a shrug; rehab with testing ends in a decision. If you are close to returning from any significant injury, ask one question of whoever is guiding you: what are my numbers, and what do they need to be?

If the answer is vague, find someone who measures. Your next season is worth an hour of testing, and the four weeks you might wait after a "not yet" are the cheapest injury insurance in sport.
