- Assessment comes before treatment, every time. A first session that puts you on a machine within ten minutes without watching you move is not physiotherapy.
- Exercise is the treatment, and hands-on work is the accelerator. Manual therapy, dry needling and taping all have real value, but as support around a progressive loading plan, never as the whole plan.
- Rest is a phase, not a treatment. For most musculoskeletal problems, prolonged rest makes the tissue less able to tolerate load, and the pain comes straight back when normal life resumes.
- Pain is not a reliable map of damage. Scan findings like disc bulges and heel spurs are common in people with no symptoms at all, so treating the image instead of the person is a well-known trap.
- Milestones beat open-ended sessions. You should know what week two, week four and week eight are meant to look like, and progress should be re-measured rather than assumed.
- Red flags get referred, immediately. Spreading numbness, loss of bladder or bowel control, night pain with fever, unexplained weight loss, or trauma with deformity go to a doctor, not to a treatment couch.
